“Oh come on!! Now it’s raining too?!” yelled Rachel, hitting the steering of her car. The mother and baby were in critical condition and she could not afford to be late. Though Sean had already been assigned to perform the surgery, Rachel would take no chance. She HAD to be present.
As she approached the bridge, a car in front careened and rammed against its edge. Rachel veered to stop her car and ran to help. She spoke as she bent to help the unconscious, pregnant woman. “I promise. Nothing will happen to you. Not under my watch. Not again.”

That is correct! She lost a patient under her care earlier and that has scarred her for life. She now seeks redemption through every person she can help.

The scene is very well set here. I wondered about the last line. Was this the lady Sean was going to operate on preceding her or did this “Not again,” refer to someone else?

Some clarification.
Rachel is a doctor under whose care a woman had lost her life. She is scarred for life as a result and now strives to seek redemption by going out of her way to help save lives. That is why she is in a hurry to get to the hospital although Sean is already assigned. It is this compulsion she displays when she says “not again”.
